Immunization Records

You can view and print your immunization records through the patient portal. If you prefer a printed copy, simply visit the front desk at any office during business hours – no need to call beforehand! If you need immunizations faxed, reach out to our medical records team via the portal, email or phone and provide the fax number.

Form Requests

Families often need forms completed by their pediatrician for camp, school, or daycare. We are happy to fill these forms out for your children in a timely manner. Please allow 7-10 business days for forms to be completed. Please include a phone number that you can be reached. Children must be up to date on their well exams to have forms completed. Forms can also be filled out at your child’s well visit.

If you have specific forms you need filled out, please fill out as much information as you can on the forms and email it to us. Alternatively, you can request one of our generic forms below:

When sending a form to be completed, please let us know how you would like to receive your form. We can upload to the patient portal, fax or have the hard copy available for pick up.

Release of Information

To request any records, please fill out a Release of Information (ROI) form and email it to us with your request. Please allow 7-10 business days for processing.
